California Healthy Kids Survey

Administered to students in grades 5, 7, 9, and 11

The district will be administering the California Healthy Kids Survey (CHKS) to students in grades 5, 7, 9, and 11 between December 6-10, 2021.

The California Healthy Kids Core Survey is sponsored by the California Department of Education.  The CHKS is a survey of resiliency, protective factors, and risk behaviors. The survey includes questions on the supports provided to children; school connectedness, climate, and safety; and risk factors such as violence and substance use.  The data collected has led to a better understanding of the relationship between students' health behaviors and academic performance, and is a critical component of school improvement efforts to help guide the development of more effective health, prevention, and youth development programs. This survey is anonymous and confidential.  The students’ privacy is protected.  No names will be recorded or attached to the survey. 

Participation is voluntary.  Students who, with parent permission, agree to participate do not have to answer any questions they do not want to answer, and may stop taking the survey at any time.  Active consent for participation will be enacted for students in grade 5. Active consent means that the parent must sign the form granting permission for their child to participate or to be excused.  Passive consent is enacted for students in grades 7, 9, and 11.  Passive consent means, parents must sign and return the form only if they do not want their student to participate.
